Discharge Care

Discharge or Hospital to Home ensures that an individual has an appropriate level of support to return home after a stay in a hospital. Our service provides a multi-clinical Health and Social Care support package designed to avoid admissions and re-admission to hospitals or care homes.

Hospital discharge will commonly only be able to take place if the individual returning home can do so safely, with the right care and support in place. It is particularly important for people with certain cognitive conditions to be able return home as soon as possible to a more familiar living environment.

At Invest in Care we are able to provide scheduled, short-notice or emergency care for individuals following time spent in hospital. We will assess the individual’s needs prior to them leaving hospital and we will liase with family and other health care professionals to ensure a smooth transition back to the home and recuperation.
